COVID 19 Vaccine Information:
- Seniors ages 65 and over are now eligible to receive the FREE COVID-19 vaccine.

- In order to receive the vaccine, you must make an appointment.
- To make an appointment you can either reach out to your medical provider or you can call either of these following numbers: 877-829-4692 or 1-833-697-4829.
- You or a family member can also schedule an appointment online at the following website: vaccinefinder.nyc.gov
- It may take a LONG time to schedule the appointment, please be patient, but persistent!

- If you have any questions, you can call your local senior center or your agency social worker.
- If you need transportation to your appointment, you can call the JCCGCI transportation department at 718-449-5000 ex. 1.
- Scammers are calling and promising New Yorkers that they will be placed on a reservation list in exchange for a fee or a deposit. This is not true. Do not share ANY financial information in regards to the Vaccine.
